- United Healthcare (we are no longer accepting new UHC patients)
Arlington Pain and Rehab DOES NOT participate with:
- Anthem – Health Keepers
- Cigna – we will look up your out of network benefits
- Medicare or Medicaid
- United Healthcare – we will look up your out of network benefits
If you do not see your plan detailed above, please contact your insurance carrier to make sure you will be covered at our offices.
Please Note: Even if we are in your plan’s network, your plan may still require a referral or pre-authorization for chiropractic or physical therapy visits. Please verify your benefits and coverage requirement with your insurance company before scheduling an appointment. Failure to obtain a referral or pre-authorization before your visit may result in coverage being denied.
NOTICE TO MEDICARE/TRICARE PATIENTS: We are not enrolled in either Medicare or TRICARE and therefore cannot submit claims for reimbursement. If Medicare or TRICARE is your primary insurance provider, you will be required to pay out-of-pocket for any services provided in our office. This applies even if we are participating with your secondary insurance provider, as any secondary insurance provider requires that a claim first be submitted to Medicare/TRICARE. You may be able to submit your own claim for reimbursement to Medicare or TRICARE, but there is no guarantee of payment.
If Medicare or TRICARE is your secondary insurance and we are participating with your primary insurance, we will only be able to submit a claim to your primary insurance and you will be responsible for any deductibles/coinsurances/copays as determined by your primary insurance benefits.
